Gray Healthcare provides a final destination solution where not only will the service user be empowered to make a choice but  where they will be equipped to live independently not simply expected to move on.

We provide this solution as we do not own any property or hospitals, but are able to source a single occupancy final destination for service users leaving their current secure environment; via a Gray Healthcare package of care.

The property selection is equally important as the package components, so risk and trigger factors are always taken into account, which allows us to correctly identify the most appropriate location and type of property for the individual.  Our dedicated property placement team work alongside our clinicians to ensure that service users are able to obtain their own tenancy that is appropriate to their clinical needs both physically and mentally, within their chosen area and takes full advantage of the housing benefit that area has to offer.

  • Personalised property selection based on environmental triggers, clinical needs and / or forensic history
  • Property sourced through private landlords or housing associations to ensure a final destination gives greater choice and quality
  • Service user is involved in the property viewings to ensure personal choice and independence
  • Initial safety inspection carried out before moving in
  • All properties will have appropriate safety checks in line with Landlords Act
  • Tenancy in service user’s name
  • Gray Healthcare act as the guarantor if required (and for the first 6 months)
  • The Gray Healthcare Property Team will arrange all aspects of relocation from hospital into their own home
  • Risk assessment carried out shortly after moving in, then every 12 months 
  • Gray Healthcare helps furnish the property and apply for appropriate community care grants
